You must submit Form IL444-1989 (Request for Replacement of Destroyed Food) to your local IDHS Family Community Resource Center (Public Aid Office) within 10 calendar days of the disaster or power outage. It can be dropped off or mailed. You can also upload it online through the ABE portal or call the IDHS Help Line (1-800-843-6154) for further assistance.
