Peoria County new families who have just delivered a baby or have a newborn in the home under 3 months old (This includes maternal visits for postpartum clients who do not have their baby with them at home, foster & adoptive parents, bereaved parents, parents with a baby in NICU, etc..)

Family Connects is a free postpartum home visiting program available to any family who resides in Peoria County. The team of Registered Nurses set up visits within a few weeks of a birth to provide assistance with postpartum screenings, educational information, mother and baby health checks, community referrals, and more.
