Eligibility
Vocational Rehabilitation: Individuals age 16 to 64 years old who have a significant physical or mental impairment that makes it difficult to go to work.
In-Home Assistance: Children and adults, age 59 and under, who have a severe disability. Please note that individuals who have a traumatic brain injury or HIV/AIDS are not bound by this age requirement. Anyone else age 60 or over in need of in-home assistance and in the City of Peoria, Marshall County, or Woodford County should contact Autonomous Case Management at 309-453-2736; and if in Peoria County (outside the City of Peoria), please contact CCSI - Case Coordination, LLC at 309-306-4550
