Eligibility
Pregnant women or families with infants (up to age 1) in Tazewell County.
Pregnant women may receive more intensive services if any of the following conditions exist:
-previous preterm birth or current health condition affecting pregnancy
-low or high pre-pregnancy weight
-less than 10th grade education
-DCFS involvement
-Domestic violence
-Homelessness
-Presence of a mental health diagnosis
-More than 4 children born in 4 years, or more than 3 pregnancies in 40 months
-Two or more miscarriages
