The veteran must contact their primary care social worker at the Bob Michel VA Outpatient Clinic for an official referral into the program.

A trained options counselor helps the veteran determine what kind of help is needed, and then VA funds are used to-
*Hire and manage an in-home assistant, including hiring a family member or friend to help with personal care and assistance around the house
*Make the home more accessible
*Order meal delivery services
