Application process

The veteran must contact their primary care social worker at the Bob Michel VA Outpatient Clinic for an official referral into the program. If there are any questions about the overall process or how the program works, please call AgeCentral.

VDC provides assistance to veterans to help them remain in their homes and is a more flexible option than traditional in-home services. A trained options counselor helps the veteran determine what kind of help is needed, and then VA funds are used to- *Hire and manage an in-home assistant, including hiring a family member or friend to help with personal care and assistance around the house *Make the home more accessible *Order meal delivery services
