Eligibility

People age 18+ with mental health and/or developmental disabilities, as well as people age 65+ older with a physical disability. If applicant has a mental health condition(s), they must be working with their doctor to manage the condition(s), depending on severity.

Required documents

Will need birth certificate, Social Security card, ID, and proof of any income.
