Application process
Bring with you: your insurance card(s) if you have coverage, all the medicines you are taking (if it is your first appointment), and immunization records and any other health documents.
If you need to cancel your visit, please call the clinic at least 24 hours before your visit.
Fees
Accepts most third-party insurance, including all Illinois Managed Care health plans, Medicare and Medicaid insurance plans. If you have no income and no insurance, no problem. A sliding fee scale program is in place and charges are based on your family size and income.

MEDICAL CARE:
-Primary health care services for all ages
-Well child care (regular checkups, school physicals)
-Lab and basic diagnostic services
-Screening for diabetes, hypertension, colon cancer, cervical cancer, and other health conditions
-Chronic disease management and support for patients with diabetes, hypertension, asthma, and other chronic conditions
-Women's health services, including breast cancer screening and Pap smears
-Care coordination
DENTAL CARE:
*Basic dental care services including: cleanings, exams, fillings, extractions.
*Periodontal (gum) treatment
*Emergency dental care
*Pediatric dentist visits for very young children
*Oral health education
