Eligibility
Criteria includes:
-Applicants should currently reside in a facility licensed by the Illinois Department of Public Health, but some exceptions apply -Must live in Illinois -Must be a U.S. citizen or legal immigrant -Must have a severe disability that will last for 12 months or longer -Cannot have more than $17,500 in non-exempt assets -Must be receiving or have applied for Medicaid benefitsRequired documents

, be between the ages of 18 and 60, have less than $10,000 in assets, have applied for Medicaid, and have a disability that will last at least 12 months or for the duration of their life.
