Anyone needing DUI evaluations or classes.

Must bring in the following before an appointment can be made: 1) Current court-purpose copy of driving record. (Needs to be within the last 6 months and include the current arrest they are being evaluated.) Can obtain from DMV with 2 forms of ID. Fee is $12.00, but subject to change. 2) Bring a copy of B.A.C. Level Sworn Report or refusal. Can obtain a copy through the Circuit Clerk of the county where you got the ticket. 3) Proof of Income 4) Previous DUI Evaluations, if applicable.
