Eligibility

Adults age 60 and above OR disabled adults between ages 18-59, and who receive services through select Illinois Home and Community Based Waiver Services Program (HCBS), or who are Medicaid and Medicare beneficiaries and receive managed care services

Home Care Ombudsmen are located throughout the state of Illinois and work to protect the rights of older persons and adults with disabilities living in the community who: are enrolled in the Medicare Medicaid Alignment Initiative (MMAI) and receive services from a managed care organization (MCO); or receive services from one of the following Home and Community Based Services (HCBS) Waiver Programs: -Persons who are Elderly -Persons with Disabilities -Persons with Brain Injury -Person with HIV or AIDS ===================== A Home Care Ombudsman can help you by answering questions you may have about the services you receive. A Home Care Ombudsman may also help you: -if you have received a denial or termination letter for in-home services, -if you are not happy with the services you are getting from your MCO or HCBS Waiver Program, and/or -if you cannot get in touch with your MCO care coordinator or HCBS Waiver Program care coordinator.
