Those 60 years of age and older who are in need of case management and in home services in the City of Peoria, Tazewell County, Woodford County, Marshall County, and Stark County
For those under 60 in need of in-home help, please contact your local Illinois Department of Rehabilitation Services.

ACM Care is a non-profit organization that serves as a Case Coordination Unit for the Community Care Program, contracted by the Illinois Department on Aging, for the City of Peoria, Tazewell County, Woodford County, Marshall County, and Stark County. ACM will initially provide a care coordinator to visit the home and conduct a comprehensive assessment to determine the needs of the person. The coordinator will then work with the person and family members to the link the person with needed services and resources. These can include in-home services, adult day care, home delivered or congregate meals, transportation options, assistive devices, and more.
