Application process
New Patients: The OSF Dental Clinic is referral-based. This means you will need a referral from a licensed provider, such as a dentist or primary care provider. Once OSF receives the referral, they will contact you to schedule an appointment.
Former Peoria City/County Health Department Dental Clinic Patients: If you were a Peoria City/County Health Department Dental Clinic patient at the time it closed and switched to OSF at the end of January 2025, you can transition to the OSF Dental Clinic by emailing [email protected] or calling (309) 679-6141.
