Eligibility

The parent or primary caregiver is an Illinois resident and is unable to secure a safe crib, bassinet, or portable play yard for newborn; AND Client is in her last trimester or baby is less than 1 month old; AND Client is uninsured, underinsured, underserved, undocumented, or is insured under Medicaid

Hours

Monday through Friday, 9 a.m. to 5 p.m.

English; and the Cribette kit itself is available in English and Spanish

Application process

If in need of a crib, please first confirm that your Medicaid plan does not offer such a service. For example, Illinois Medicaid providers such as Blue Cross Blue Shield and Meridian have historically covered a crib or Cribette. If your plan does not cover this, then please speak with a healthcare professional, faith-based organization, family case manager, or another service provider you are working with and direct them to the SIDS of Illinois Crib Referral Service. This provider will complete the application (available via the website in this profile). Please note that application CANNOT be completed by the person seeking the crib. After the application is recieved and approved by SIDS of Illinois, you will be assigned a Health Educator (HE). The HE will provide education over the telephone that covers topics including Safe Sleep, Tummy Time, Breastfeeding, Smoking Cessation, Pregnancy Spacing, Importance of Post-Partum Care, Post-Partum Blues/Anxiety/Depression, and Car Seat Safety. Following the completion of this education, the Cribette kit will be shipped. Someone in the household must sign for the package upon its arrival.
