New Patients: The OSF Dental Clinic is referral-based. This means you will need a referral from a licensed provider, such as a dentist or primary care provider. Once OSF receives the referral, they will contact you to schedule an appointment. Former Peoria City/County Health Department Dental Clinic Patients: If you were a Peoria City/County Health Department Dental Clinic patient at the time it closed and switched to OSF at the end of January, you can transition to the OSF Dental Clinic by emailing [email protected] or calling (309) 679-6141.
