Eligibility
Vocational Rehabilitation: Individuals age 16 to 64 years old who have a significant physical or mental impairment that makes it difficult to work.
In-Home Assistance: Children and adults, age 59 and under, who have a severe disability. Anyone age 60 or over in need of in-home assistance should contact Autonomous Case Management at 309-453-2736.
