The Graham Health System mental/behavioral health care staff includes Psychiatric Mental Health Nurse Practitioners (PMHNP), a Licensed Clinical Practice Counselor (LCPC), and a Licensed Clinical Social Worker (LCSW). These professionals provide support to adults, teens, and children seeking diagnosis, short or long-term treatment (including medication, when appropriate), and management of mental and emotional health challenges. Services are offered, by appointment, at the Canton, Elmwood, Farmington, Macomb, and Lewistown locations.

Advocates for Access provides housing search and information in two ways: -Acts as a screening agent for individuals and families on Medicaid to be placed on the Statewide Referral Network (SRN) list for subsidized housing. The SRN list allows a person or family to pick several counties, even specific cities, in which they are interested in residing. When an opening becomes available, they are notified and may apply to the housing entity or complex. This is not guaranteed housing but a means to find out about available housing. -Maintains lists of subsidized and other housing by county (Peoria, Tazewell, Woodford, and Fulton), available on the Advocates for Access website

The Living Room Program at Trillium Place Crisis Center, an affiliate of Carle Health, provides 24/7 walk in mental health services for individuals experiencing a mental health or substance use disorder crisis. Services offered include support by certified recovery specialists, assistance with problem solving and goal settings, crisis intervention and safety planning, linkage to referrals, such as food, housing and healthcare services, and healthy refreshments. To submit a claim for actual damages, including food spoilage, due to a power interruption, outage, surge, or other fluctuation, complete a Claim Form (link on this page) and mail the Claim Form and supporting documentation to: Brentwood Services Administrators, Inc. Ameren Illinois will send the customer written confirmation that it received the customer’s Claim Form shortly after receiving a claim. 2. Ameren Illinois will determine whether the claim arises from a qualifying interruption, outage, surge, or other fluctuation. Ameren Illinois will determine whether it believes any of the statutory exemptions apply. If none of the exemptions applies, the claim will be processed. If one or more of the exemptions apply, then Ameren Illinois may deny the claim or request a waiver from the Illinois Commerce Commission (ICC). Ameren Illinois will inform the customer of the waiver request, and that the processing of the claim will be stayed until the ICC makes a decision. 1. If the ICC grants Ameren Illinois’ waiver request, then Ameren Illinois will inform the customer that it is not liable for the customer’s claim. 2. If the ICC denies Ameren Illinois’ request and the decision is not appealed, then Ameren Illinois will process the customer’s claim and provide additional notice to customers who may have been affected by the qualifying event. 3. Ameren Illinois will communicate its proposed resolution of the claim to the customer. Typical claims are resolved within 90 days (excluding any time that claims were held pending an ICC or court decision) after Ameren Illinois has received a completed Claim Form with the required proof of damage.
