ACM Care is a non-profit organization that serves as a Case Coordination Unit for the Community Care Program, contracted by the Illinois Department on Aging, for the City of Peoria, Tazewell County, Woodford County, Marshall County, and Stark County. ACM will initially provide a care coordinator to visit the home and conduct a comprehensive assessment to determine the needs of the person. The coordinator will then work with the person and family members to the link the person with needed services and resources. These can include in-home services, adult day care, home delivered or congregate meals, transportation options, assistive devices, and more.

Children's Home offers a family focused, strengths-based care coordination and wrap-around service for youth with complex behavioral health needs. Children's Home is the Care Coordination and Support Organization (CCSO) for Pathways to Success in Central Illinois, serving as a centralized leader with a dedicated and trained team of coordinators that offer support to clients and families. Pathways is a program for Medicaid-enrolled youth under the age of 21 in Illinois who have complex behavioral health needs and could benefit from additional support. Care coordinators collaborate closely with community services to help build supports in the home, school, and community. The goal of the Pathways program is to improve family functioning, reduce caregiver stress, provide stability for at-risk youth in the home, improve school attendance, and reduce hospitalization. The Salvation Army is an international movement and an evangelical part of the universal Christian Church. It offers numerous assistance programs for those in need. ------------------ 2026 Warming & Cooling Center Information: Open as a warming or cooling center during extreme temperatures. The Salvation Army's building is at 243 Derby St. in Pekin, and The Pekin Outreach Initiative is housed on the East Side of this building. The Pekin Outreach Initiative portion is open from 11am-4pm, Monday through Sunday, for people to drop in and warm up/cool off. The Salvation Army shelter in the gym at the same location is open overnight 5pm-10:30am, Monday through Sunday, for warming or cooling. Usually limited to 10-15 people at most for this purpose.

Peoria Jobs Partnership is a Christian mentoring program for people who might be experiencing problems getting gainful employment. For the general public, there is the Community Program. The curriculum includes 12 classes, taught by different community professionals over a 12-week period. Students are required to complete homework for each class, and are awarded a certificate of completion at the end of the program. The Community Program allows students access to great instructor/ practitioners, and allows them contacts with community leaders that can act as advocates for them when they seek employment. Additional programs, such as Solar Training and the Fresh Start Workshop, are offered for individuals with a felony in their background and those re-entering society from incarceration. For individuals representing themselves in a civil legal case, volunteer attorneys are available for one-time, 30 minute online consultations to answer questions, explain legal procedures, and provide general direction on the case. Volunteers are only available to provide brief and one-time advice and information. They are not able to provide representation in court for the legal matter. The help desk is not able to assist with matters involving domestic violence, or on emergency petitions or orders.

The FOC tailors’ the unique financial situation of citizens to the resources required to make a financial “Fresh Start.” The program, supported by LISC, helps low-to-moderate income families and individuals achieve financial stability by implementing the integrated services approach. This approach is a new way to offer four core services: 1. Financial coaching and education to help build assets 2. Employment and career development 3. Better access to public benefits 4. Life skills training This strategy creates new opportunities for individuals and helps connect employers to well-prepared workers. Open to all Peoria and Tri-County residents.

Provides one-time financial assistance grants (up to $2,500) to eligible active-duty service members, activated Guard or Reserve members, and their families who are experiencing unexpected financial hardship due to deployment, military-related injury, or service-related circumstances. Assistance may help cover basic living expenses such as rent, utilities, or other essential bills. Grants are paid directly to creditors and do not require repayment. Expenses Eligible for consideration of payment (daily necessities): *Housing expenses - mortgage, rent, repairs, and homeowners’ insurance (establishing residence/insurance or paying off expenses for a residence that is no longer being occupied will not be considered) *Vehicle expenses - payments, repairs, insurance (purchasing/paying off loan, major repairs for vehicles over 10 years old, registering, establishing insurance will not be considered) *Utilities (establishing/paying off utilities for a residence no longer being occupied will not be considered) *Childcare *Medical bills (the patient’s portion for necessary or emergency medical care only).
