Phoenix Community Development Services' PATH program provides outreach and support to individuals in Peoria, Tazewell, Woodford, and Fulton counties who are experiencing homelessness and have a serious mental illness and/or substance abuse condition.

Community Needs Agency in Chillicothe is an all-volunteer resource center that serves residents of Illinois Valley Central School District 321 (Chillicothe, Rome, Mossville area). All programs and services are income-based. A couple of service clarifications: -Emergency food can be provided as available to clients by appointment only. -One aspect of energy assistance is LIHEAP application assistance, offered by appointment only for clients.

The Fulton County Health Department serves as a site for the Illinois Breast and Cervical Cancer Program for Fulton, McDonough, and Schuyler counties. The Illinois Breast and Cervical Cancer Program offers free mammograms, breast exams, pelvic exams and Pap tests to eligible women. Even if a woman has already been diagnosed with cancer, she may receive free treatment through the program if she qualifies.

The Woodford County Veterans Assistance Commission (VAC) is operated by and for U.S. veterans. VAC officers work with veterans and dependents to determine their entitled benefits and file claims with the United States Department of Veterans Affairs. VAC officers guide applicants through the entire process and fight to ensure a fair review of all claims. The Veterans Assistance Commission's programs are considered emergency and interim financial assistance programs and should not be considered as an ongoing financial supportive program. All assistance provided by the VAC is predicated on an established standard of need as set forth by the Illinois Department of Human Services (IDHS). It is not the intent of either the Commission or the County to provide assistance either less than or in excess of the levels of assistance adopted by IDHS.

Core programs include- *Financial Stability Program The purpose of the Financial Stability program is to help participants to increase their revenue while decreasing their expenses, thus helping them to improve their quality of life. This is a case management program that provides financial, credit and employment education, training, coaching and mentorship while providing basic needs like food, clothing, transportation, healthcare, and hygiene products. *Citizenship Classes Includes assistance with application process & Individual Attention/Support. Completely free. *La Familia La Familia Program that strives to restore dignity and protect our local Latino community by providing culturally responsive and healing support across the lifespan, from children to seniors. *S.T.E.A.M.S Academy An after-school program for K-8th graders that focuses on increasing learning potential with a concentration on one-on-one tutoring, enrichment activities and service learning opportunities. It features the S.T.E.A.M.S curriculum, which includes First Lego League, allowing children to research, design, and build Lego Robot. The goal is that the children grow to love learning and look beyond simply passing their school courses, but dream to excel far beyond. When school is out for the summer, there is an eight-week Summer Camp program focusing on leadership development that provides four consecutive 2-week camp themes with an enriching diverse program in a fun and safe environment. The children learn about healthy living, giving back to one's community through service learning, and social and emotional well-being. *Pathway to YOUR Future Designed specifically to prepare, teach, and support high school students' transition into adulthood. The goal is to be able to provide high school students with resources to develop life skills and different course certification opportunities to increase post-grad jobs right out of high school. *Parent Mentor Program Parent Mentors volunteer in classrooms for 2 hours/day for at least 100 hours. The Parent Mentors are supported by weekly training on classroom instructional practices and leadership training. As a result, they support each other to pursue their goals and unite the school community for long-term change. *Peoria Peacekeepers The Peoria Peacekeepers Network is a referral program established by the Peoria Friendship House that focuses on using a restorative justice approach to provide juvenile offenders who have admitted to committing certain eligible offenses, misdemeanors, or felony. Juvenile offenders are given the opportunity to make restitution, connect with the community, and receive needed services and support. Peoria Friendship House works in tandem with various organizations to provide aid to juvenile offenders via referral sources such as the Peoria Police Department Station Adjustments, Peoria Public Schools discipline referrals, and Peoria County Probation Informal Supervision.

Trillium Place Access Center, an affiliate of Carle Health, provides information and linkage to Trillium Place mental health and substance use programs and services located in Central Illinois. Trillium Place provides comprehensive mental health and substance use disorder services. Mental health outpatient services include counseling, community-based services, and psychiatry. Substance use disorder programs include inpatient, outpatient, medication assisted treatment, and detoxification services. Proudly affiliated with Carle Health, Trillium Place is a not-for-profit organization originally formed in April 2019 by joining UnityPoint Health Behavioral Health Services, the Human Service Center, and Tazwood Center for Wellness. The Breastfeeding Resource Center offers a variety of services and support programs to make breastfeeding work for moms and families. Donor breast milk is available for purchase at the OSF Breastfeeding Resource Center. All breast milk has been pasteurized and is safe for your baby. And, in collaboration with the Mothers' Milk Bank of the Western Great Lakes, donor breast milk is also available in an emergency. Emergency donor milk can be purchased 24/7 at the Labor and Delivery Triage Desk on the fifth floor at OSF Saint Francis, (530 NE Glen Oak Avenue, Peoria, Illinois 61637).

Sun Meals To Go is a federal program sponsored by the USDA. ANY family with children aged 1-18 qualifies for this program. There is NO income limit or range. The program offers 14 free shelf-stable meals per child per week. Families need to register and choose a pick-up location that suits their needs. Pick-up locations are available in Eureka and Spring Bay in Woodford County and in Green Valley in Tazewell County. Participants do not need to reside in the pick-up location counties to be eligible. Resource Link offers guidance on finding mental health care providers, including those outside of the OSF network, who serve children and adolescents. This service is open to the general public. The program also offers mental health training to physicians and staff, physician to physician consultation, and care coordination.
