Home for All Continuum of Care's Coordinated Entry program serves as the sole channel for accessing the Rapid Re-Housing and Permanent Supportive Housing interventions for people experiencing HUD-defined homelessness. Trained Coordinated Entry assessors enter individuals and families into the Homeless Management Information System and complete a vulnerability assessment with the client(s). If the individual or family is found to be at advanced vulnerability in their homelessness based on the assessment, they are referred to the Coordinated Entry list. When a Rapid Re-Housing or Permanent Supportive Housing program has availability, a representative from the respective program accesses the list to pull next referral for the opening. Coordinated Entry prioritizes the most vulnerable clients experiencing homelessness for a limited amount of federal and state-funded resources that flow through Home for All Continuum of Care. Anyone whose situation is not determined to be a fit for Coordinated Entry will be provided with other housing resources, as will those who are referred into the Coordinated Entry pool so that they have additional options to pursue.

The OSF Children's Hospital of Illinois offers comprehensive child passenger safety seat support, a program for low-cost child passenger safety seats, and serves as a Special Needs Child Passenger Safety Resource Center.

Woodford County Heartline exists to provide financial aid and essential basics for residents of Woodford County who find themselves in a short-term crisis.

The Community Well in El Paso offers the El Paso Food Pantry.

FamilyCore offers counseling for those who are experiencing emotional difficulties or conflicts concerning their role as parents.

Rebuilding Together Peoria is an organized volunteer effort that rehabilitates the homes of eligible city of Peoria, East Peoria, Richwoods Township, and Peoria County residents, particularly the elderly or disabled, who cannot maintain their homes. Projects range from yard clean up to more extensive renovations involving plastering, painting, carpentry, plumbing, and electrical repairs. The organization typically completes projects on 15 homes or non-profit facilities each year.

Schlarman House, located in Peoria, Illinois, offers 75, one-bedroom, income-based apartments for senior citizens (age 62+) and individuals age 18+ with a disabling condition. Units are cable ready and have an emergency call system, air conditioning, and off street parking. Residents at Schlarman House have access to a community room, television room, on-site laundry facility and meal progams. Water and sewer, lawn care, and maintenance also included.

DCFS offers an advocacy office for children and families through their state government complaints and ombudsman office.

The Division of Rehabilitation Services is the state's lead agency serving individuals with disabilities. DRS works in partnership with people with disabilities and their families to assist them in making informed choices to achieve full community participation through employment, education, and independent living opportunities.

Brightside Recovery in provides comprehensive treatment services to address the underlying causes of addiction alongside the physical symptoms. In addition to in-person services, Brightside offers completely remote treatment via telehealth, providing drug screens, medication delivery, and virtual counseling. Treatment programs include those focusing on fentanyl, heroin, and Kratom (70H) addiction, as well as transitioning off of methadone. Medication-assisted treatments (MAT) available include Suboxone, Sublocade, Vivitrol, and Brixadi.

Provides one-time financial assistance grants (up to $2,500) to eligible active-duty service members, activated Guard or Reserve members, and their families who are experiencing unexpected financial hardship due to deployment, military-related injury, or service-related circumstances. Assistance may help cover basic living expenses such as rent, utilities, or other essential bills. Grants are paid directly to creditors and do not require repayment. Expenses Eligible for consideration of payment (daily necessities): *Housing expenses - mortgage, rent, repairs, and homeowners’ insurance (establishing residence/insurance or paying off expenses for a residence that is no longer being occupied will not be considered) *Vehicle expenses - payments, repairs, insurance (purchasing/paying off loan, major repairs for vehicles over 10 years old, registering, establishing insurance will not be considered) *Utilities (establishing/paying off utilities for a residence no longer being occupied will not be considered) *Childcare *Medical bills (the patient’s portion for necessary or emergency medical care only). Expenses Ineligible for consideration for payment: *Military debt/repayment, tuition assistance debt/repayment *Cable, internet, secondary phones *Taxes owed of any kind *Rentals of any kind (vehicle, furniture, and/or electronics) *Any other expenses not determined to be a "daily necessity" The eligible and ineligible expense lists are not all inclusive. Each case will be carefully reviewed for its own merits. All applications are individually reviewed, and the VFW reserves the right to make exceptions on a case by case basis.

The Veterans Assistance Commission is an agency that is operated by and for veterans. The commission provides aid to those indigent veterans and their families residing in Tazewell County who meet eligibility requirements.

OSF Illinois Neurological Institute provides evaluation, treatment, and ongoing care for a wide range of neurological conditions. Services include specialized clinics for concussion, brain tumors, epilepsy, headaches, memory disorders, multiple sclerosis, neuromuscular conditions, neuro-ophthalmology, balance and vestibular disorders, Parkinson’s disease and other movement disorders, pediatric neurosurgery, sleep disorders, spine care, and stroke. The institute also offers physical medicine and rehabilitation to support recovery and long-term management.

Whole Life Solutions, LLC offers a wide range of counseling options.

The Lions of Illinois Foundation is occasionally able to assist Illinois residents in need of eye glasses and/or hearing aids.

Akron Township Office offers general relief fund to their residents.

The Illinois School for the Deaf offers comprehensive assessment, enrichment, intervention, and educational services for school-age residents of Illinois who are deaf and hard of hearing.

The Health & Wellness Connection of Fulton County (HWCFC) is a care management facility that works closely with Graham Health System to provide qualified individuals with low or no cost health services. Individuals who need assistance with things like medications, diabetic & other medical supplies, vision and dental care, and transportation for medical care may qualify for financial assistance. Individuals who do not qualify may seek assistance for referrals to other appropriate organizations.

The Family Advocacy Center provides of case management, mentoring, intensive parent coaching, and overall guidance for families navigating the child welfare system or otherwise in need of support. The Center also offers anger management for adults. Occasionally, one-day sessions are available for teens.

The Fulton County Health Department offers a variety of public health services. Also, there is a Free Overdose Prevention Vending Machine located just inside the front doors, available during normal business hours (Monday through Friday, 8 a.m. to 4 p.m.)

Addus offers in-home health and in-home care services. In-home care services include- Bathing Dressing Grooming Meal preparation Housekeeping Errands/transportation Medication reminders Supportive services can be arranged for any frequency of time, from one to twenty-four hours of care daily, or on a respite or temporary basis.

Hand Up Peoria offers a food pantry twice per month (2nd & 4th Thursday, usually), as well as a hygiene item pantry (2nd & 4th Wednesday, usually). Thursday pantry items include food, meat, some refrigerated items and fruits as available, and bread. Wednesday hygiene items include incontinence supplies, feminine hygiene items, baby needs (diapers & wipes, primarily), and other hygiene products.
