Heartland Health Services is a federally funded organization that provides medical, mental health, and substance use services for everyone, including low-income adults and children, regardless of ability to pay. One of the focuses of the Illinois Respite Coalition (IRC) is supporting a safety net for caregivers in crisis. The IRC has Emergency Respite funding stipends for caregivers across Illinois in a planned or unplanned emergency. This program provides funding so caregivers can pay for services they choose. Crisis situations that may qualify for the Emergency Respite Program include, but are not limited to the following circumstances: -Caregiver illness (physical, mental, and/or emotional) -Caregiver hospitalization or doctor appointments -Illness of a loved one -Funeral or wake -Drug or alcohol abuse counseling or support -Care recipient transitions (i.e. living arrangements) -Loss of employment or work related situations -Declining Mental Health

The Share Closet is a community resale store carrying a variety of clothing, kitchen supplies, furniture, decor, toys, books, and much more. The Share Closet also collects medical supplies, bedding, kitchen, and baby supplies to give to those in need. The site operates as a thrift store but provides certificates for individuals who cannot afford needed items.

North Central Behavioral Health Systems, Inc. is a non-profit behavioral health care organization serving La Salle, Bureau, Marshall, Putnam, Fulton, McDonough, and Grundy counties.

Carle Health Methodist Hospital offers outpatient behavioral health services at its Methodist location for children, adolescents, adults, and families on Medicaid.

The Bureau, Putnam, & Marshall County Health Departments work to protect the health of the citizens of their counties through education, collaboration, and public service.

The Living Room Program at Trillium Place Crisis Center, an affiliate of Carle Health, provides 24/7 walk in mental health services for individuals experiencing a mental health or substance use disorder crisis. Services offered include support by certified recovery specialists, assistance with problem solving and goal settings, crisis intervention and safety planning, linkage to referrals, such as food, housing and healthcare services, and healthy refreshments. Proudly affiliated with Carle Health, Trillium Place is a not-for-profit organization originally formed in April 2019 by joining UnityPoint Health Behavioral Health Services, the Human Service Center, and Tazwood Center for Wellness. With locations in Peoria, East Peoria, Pekin, Lincoln, and Eureka, Trillium Place provides a comprehensive range of mental health and substance use disorder services.

The Fulton County Health Department serves as a site for the Illinois Breast and Cervical Cancer Program for Fulton, McDonough, and Schuyler counties. The Illinois Breast and Cervical Cancer Program offers free mammograms, breast exams, pelvic exams and Pap tests to eligible women. Even if a woman has already been diagnosed with cancer, she may receive free treatment through the program if she qualifies.

Tazwood Community Services, Inc. is the community action agency now serving Peoria County, in addition to Tazewell and Woodford counties.

The Money Management Program is a volunteer program that matches trained and bonded volunteers with seniors who need help managing their finances. The volunteers can organize and pay bills, establish and maintain budgets, assist in applying for financial programs and assist with setting and achieving financial goals.

Snyder Village offers a retirement community, as well as assisted living, nursing care, skilled therapy, rehabilitation, and home care.

To date AGSF has donated $10,000.00 in scholarships and grants supporting families of the fallen.

Age Central is a private, not for profit service agency serving seniors, individuals with disabilities, family caregivers, and grandparents raising grandchildren. One of 13 Area Agencies on Aging in Illinois and one of over 728 in the United States, CIAA is authorized by the Older Americans Act of 1965 as amended, Illinois Act on Aging, and the Illinois Department on Aging to provide services in Fulton, Marshall, Peoria, Stark, Tazewell, Woodford counties.

The Aging and Senior Assistance Helpline acts as a portal to various services offered by the Illinois Department on Aging, including inquiring about senior health insurance (Medicare-related questions), checking Benefit Access application status, and more. The line also accepts reports for the Long-Term Care Ombudsman Program, which investigates mistreatment and poor living conditions in long-term care facilities. This service is open to individuals age 18 or older who are either a current resident, a prospective resident, or a former resident of a long-term care facility.

The Peoria Friendship House is a faith-based organization reaching out to all those in need, regardless of faith. Core programs include- *Financial Stability Program The purpose of the Financial Stability program is to help participants to increase their revenue while decreasing their expenses, thus helping them to improve their quality of life. This is a case management program that provides financial, credit and employment education, training, coaching and mentorship while providing basic needs like food, clothing, transportation, healthcare, and hygiene products. *Citizenship Classes Includes assistance with application process & Individual Attention/Support. Completely free. *La Familia La Familia Program that strives to restore dignity and protect our local Latino community by providing culturally responsive and healing support across the lifespan, from children to seniors. *S.T.E.A.M.S Academy An after-school program for K-8th graders that focuses on increasing learning potential with a concentration on one-on-one tutoring, enrichment activities and service learning opportunities. It features the S.T.E.A.M.S curriculum, which includes First Lego League, allowing children to research, design, and build Lego Robot. The goal is that the children grow to love learning and look beyond simply passing their school courses, but dream to excel far beyond. When school is out for the summer, there is an eight-week Summer Camp program focusing on leadership development that provides four consecutive 2-week camp themes with an enriching diverse program in a fun and safe environment. The children learn about healthy living, giving back to one's community through service learning, and social and emotional well-being. *Pathway to YOUR Future Designed specifically to prepare, teach, and support high school students' transition into adulthood. The goal is to be able to provide high school students with resources to develop life skills and different course certification opportunities to increase post-grad jobs right out of high school. *Parent Mentor Program Parent Mentors volunteer in classrooms for 2 hours/day for at least 100 hours. The Parent Mentors are supported by weekly training on classroom instructional practices and leadership training. As a result, they support each other to pursue their goals and unite the school community for long-term change. *Peoria Peacekeepers The Peoria Peacekeepers Network is a referral program established by the Peoria Friendship House that focuses on using a restorative justice approach to provide juvenile offenders who have admitted to committing certain eligible offenses, misdemeanors, or felony. Juvenile offenders are given the opportunity to make restitution, connect with the community, and receive needed services and support. Peoria Friendship House works in tandem with various organizations to provide aid to juvenile offenders via referral sources such as the Peoria Police Department Station Adjustments, Peoria Public Schools discipline referrals, and Peoria County Probation Informal Supervision.
