PACE is a Medicare and Medicaid risk-based care program that offers comprehensive, team-based medical and social services for seniors through collaborating health care agencies, allowing older older adults to age at home (as opposed to entering a nursing facility). PACE also brings relief to the growing number of family caregivers who are meeting the health care and social needs of older adults every day.
