The Division of Specialized Care for Children (DSCC) is the Illinois Title V agency that provides care coordination for families and children with special health care needs. DSCC, governed by Administrative Rules, helps children with disabilities, and those who have conditions which may lead to disabilities, grow and develop to the full extent of their abilities. DSCC Care Coordinators can support families with the following needs: *Paying for certain medical expenses *Accessing diagnostic tests *Finding specialized medical care *Using insurance & All Kids / Medicaid *Finding resources & information *Developing a care coordination plan Communicating with doctors, specialists and schools *Preparing for transition

First Hand Foundation is a public 501(c)(3) organization that provides funding for individual children—both domestically and globally—who need assistance with clinical necessities (such as surgery, medication and therapy), medical equipment and travel related to care.
