Cordoba Healthcare works with local doctors, nurses, community volunteers, and students to provide a free specialty medical clinic model. Areas of specialization include cardiology, gastroenterology and liver, nephrology (kidney disease), pulmonary, basic dental services (depending on situation), and more. Cordoba collaborates with Heartland Health Services and local hospitals to receive referrals for specialty treatment, but also accepts self-referrals from patients. The Multiple Sclerosis Foundation is a nonprofit serving people affected by multiple sclerosis (MS). Callers receive personalized information about multiple sclerosis and connections to local and national resources related to MS. Service coordinators and peer counselors can talk through needs, answer questions, and help find the right support. Through the helpline, people with MS can also learn about and apply for a variety of support programs (eligibility usually requires an MS diagnosis), such as *Emergency Assistance Grant -- financial help for urgent needs like rent or utility bills (documentation required) *Healthcare Assistance Grant -- helps uninsured individuals pay for neurologist or dental visits *Homecare Assistance Grant – short-term homecare, caregiver respite, therapy services *Transportation Assistance Grant – funding for paratransit or rides to medical appointments (Lyft/Uber) and minor car repairs *Assistive Technology Program – info and partial funding for devices that aid daily living (e.g., canes, adaptive tech) *Computer Program – provides computers to help people with MS stay connected. *Cooling Program – cooling vests and accessories for heat sensitivity. In addition, the helpline can connect callers to peer counseling, advice on local support groups, and online communities.

Mindset Care specializes in assisting individuals with mental health diagnoses, developmental and physical disabilities, chronic illnesses, those in critical care situations, and their caregivers in applying for Social Security Disability benefits - SSI and SSDI. Assistance with the SSI/SSDI appeal process is also offered. Mindset can support individuals ages 0-63 in all 50 states. The process blends technology with legal advocacy for a streamlined approach to accessing benefits.

Children Need their families and families need their children. Intact Family Services is a voluntary program designed to keep each family intact when the threat of separation is real. This program works with at-risk families who have been referred for continuing assistance and monitoring following a child abuse and neglect investigation with the Department of Children and Family Services (DCFS). The goals of this program are to enable children to remain safely in their home, make reasonable efforts to keep families together, address and resolve the issues that place the children at risk, and to avoid having children separated from their family. Children's Home Association of Illinois caseworkers will help the family develop a short-term, agreed upon plan to make the changes that will help keep children safe. They will act as a support system and resource without judgement, working to ensure all children in the home are safe and supported. Medicaid fraud occurs when a Medicaid provider knowingly makes, or causes to be made, a false or misleading statement or representation to obtain reimbursement from the medical assistance program. This would include, but is not limited to: doctors or other health professionals billing for services they did not provide or allowing uncertified or unqualified individuals to provide services, medical supply companies billing for products they did not provide, dentists performing unnecessary teeth extractions, charging Medicaid more than the reasonable value of the services and providing services that were medically unnecessary. The Illinois Attorney General’s Medicaid Fraud Control Unit (MFCU) investigates and prosecutes such fraud.
