ACM Care is a non-profit organization that serves as a Case Coordination Unit for the Community Care Program, contracted by the Illinois Department on Aging, for the City of Peoria, Tazewell County, Woodford County, Marshall County, and Stark County. ACM will initially provide a care coordinator to visit the home and conduct a comprehensive assessment to determine the needs of the person. The coordinator will then work with the person and family members to the link the person with needed services and resources. These can include in-home services, adult day care, home delivered or congregate meals, transportation options, assistive devices, and more.

Family Connects is a free postpartum home visiting program available to any family who resides in Peoria County. Provides one-time financial assistance grants (up to $2,500) to eligible active-duty service members, activated Guard or Reserve members, and their families who are experiencing unexpected financial hardship due to deployment, military-related injury, or service-related circumstances. Assistance may help cover basic living expenses such as rent, utilities, or other essential bills. Grants are paid directly to creditors and do not require repayment. Expenses Eligible for consideration of payment (daily necessities): *Housing expenses - mortgage, rent, repairs, and homeowners’ insurance (establishing residence/insurance or paying off expenses for a residence that is no longer being occupied will not be considered) *Vehicle expenses - payments, repairs, insurance (purchasing/paying off loan, major repairs for vehicles over 10 years old, registering, establishing insurance will not be considered) *Utilities (establishing/paying off utilities for a residence no longer being occupied will not be considered) *Childcare *Medical bills (the patient’s portion for necessary or emergency medical care only). Trained Helpline Specialists answer questions, provide support, and connect the caller with services in Illinois. ---------------------------------------- The Illinois Helpline also offers medication on demand to IL residents seeking treatment for opioid use disorder (OUD) and alcohol use disorder (AUD). Medication-assisted recovery (MAR) involves the use of FDA-approved medications for treatment of OUD and AUD. Now an individual can speak directly with a provider over the phone, immediately receive a prescription or expedited, in-person appointment, and get transportation assistance to the pharmacy or clinic- all initiated through one phone call. Callers can access MAR NOW 24 hours a day, 7 days a week. Care managers help callers determine their best treatment options and connect them to a provider for an immediate telephone appointment and medication prescription. Care coordinators can also facilitate a same- or next-day, in-person appointment. All callers are connected to ongoing treatment with a community provider that best meets their needs.

The Healthier Homes Initiative is a no-cost program providing eligible Ameren Illinois residential customers with home upgrades and minor repairs to improve safety, comfort, indoor air quality and overall home health. Eligible customers may receive energy efficiency improvements to their home such as air sealing, insulation, lighting replacement, ventilation and moisture control measures, and other safety measures. Upgrades and minor repairs are based on an assessment conducted by qualified contractors.
