ACM Care is a non-profit organization that serves as a Case Coordination Unit for the Community Care Program, contracted by the Illinois Department on Aging, for the City of Peoria, Tazewell County, Woodford County, Marshall County, and Stark County. ACM will initially provide a care coordinator to visit the home and conduct a comprehensive assessment to determine the needs of the person. The coordinator will then work with the person and family members to the link the person with needed services and resources. These can include in-home services, adult day care, home delivered or congregate meals, transportation options, assistive devices, and more.

The Gateway Foundation's Peoria location provides outpatient psychiatric care and substance use treatment (and linkage with/transportation to inpatient treatment at their other Illinois locations) for people age 18+. Additionally, the location offers mental health services to patients who may require individual therapy or medication management, regardless of whether or not they have a substance use condition.

The person/household will have to apply for township general assistance first; and if they do not qualify, they will then be screened for emergency assistance. Heartland Health Services has been in operation since February 1, 2004, as a federally designated community health center. Its targeted population is low-income adults and children. Among its many programs, it provides health insurance, Medicaid, and Medicare navigation; as well as help in completing Social Security benefits applications, SNAP/Link applications, and applications for various assistance programs (utilities, etc.).
