The Health & Wellness Connection of Fulton County (HWCFC) is a care management facility that works closely with Graham Health System to provide qualified individuals with low or no cost health services. Individuals who need assistance with things like medications, diabetic & other medical supplies, vision and dental care, and transportation for medical care may qualify for financial assistance. Individuals who do not qualify may seek assistance for referrals to other appropriate organizations.

ADDWC offers a variety of residential services which promote independent living and are based upon an individual's needs. Twenty-four supports are offerered in a small, group home setting (4-8 individuals). For those who wish to remain in their home, staff can provide intermittent supports including assistance with banking, shopping, community access and daily living skills. All residential services are funded through the Department of Human Services.

The Moonlight Coalition for Adult Learning focuses on providing easy access to GED education for those seeking to earn their high school diploma. At this time, Moonlight provides GED and adult literacy education at Logan Recreation Center, along with other locations as needed. Individuals can drop into a class at any time. There is no cost at all to attend these classes and all supplies are provided. Moonlight Coalition honors its graduates with a cap and gown graduation ceremony twice a year.

The office is able to provide expert guidance on Social Security questions. Among the case management services offered by Living by Your Design are one on one case management with a nurse, as well as bill paying assistance.

Patients will learn new coping skills, recovery strategies, build interpersonal skills, and develop social support networks. Accelerated Resolution Therapy ART-Accelerated resolution therapy, is a type of therapy that combines principles from several traditional forms of psychotherapy to reduce the effect of trauma and other psychological stressors by using techniques such as rapid eye movement and image rescripting. The Restaurant Employee Relief Fund is a program of the Illinois Restaurant Association Educational Foundation (IRAEF) that offers emergency assistance to Illinois food and beverage workers facing an unanticipated hardship within the past 90 days such as: accident, illness, injury, death of an immediate family member, or natural disaster. If you are in need of assistance, please complete the online application form. All grant requests will be reviewed. The IRAEF Restaurant Employee Relief Fund grants are awarded to eligible individuals based on available funds and limited to one per individual. Grants cover costs of living expenses for life-altering events and range from $250-$3,000.
