The Living Room Program at Trillium Place Crisis Center, an affiliate of Carle Health, provides 24/7 walk in mental health services for individuals experiencing a mental health or substance use disorder crisis. Services offered include support by certified recovery specialists, assistance with problem solving and goal settings, crisis intervention and safety planning, linkage to referrals, such as food, housing and healthcare services, and healthy refreshments. Older persons will have their medications screened to identify medication-related problems and errors that may endanger the lives who participate in health promotion programs. This service is provided by trained staff utilizing a computer system and appropriate data collection materials working in conjunction with a pharmacist or other medication expert. The program consists of individualized screening, assessment, and alert process to identify medication problems.

CCSI - Case Coordination, LLC is a non-profit organization that serves as a Case Coordination Unit for the Community Care Program, contracted by the Illinois Department on Aging, for Peoria County (ACM Care covers the City of Peoria) and Fulton County. CCSI will initially provide a care coordinator to visit the home and conduct a comprehensive assessment to determine the needs of the person. The coordinator will then work with the person and family members to the link the person with needed services and resources. These can include in-home services, adult day care, home delivered or congregate meals, transportation options, assistive devices, and more.
